This package includes:

1. Transfer from place of death to our facility,

2. Refrigeration,

3. Disposition by Cremation,

4. OSHA required cremation “alternative” container (this is not an urn),

5. Death Certificates (10).

Additional items and services may be added to the above package upon request. Any additional services requested that carry an additional charge will be explained in advance.

In order to select this package, once the services have been paid for and death certificate information has been input, please ask your medical professional to call our office at 713-802-0000 to inform us of the death.
